Care Plan Coordinator

Hooray are proud to be supporting a local charity to assist in the recruitment for a Care Plan Coordinator for one of their homes in Stroud. The home is rated ‘good’ in all areas with the CQC.
As a care plan coordinator, you will assist in the management of care planning across the home, providing guidance to the care team and promoting a personalised care plan for the residents. You will liaise with the Home Managers / Team Leader and Senior Carers in connection with personalised care planning.
You will attend pre assessment meetings recording accurate information on new residents, liaise with the nurse and team leader following clinical appointments and work with new starters to ensure they learn the best practice of documentation whilst promoting person centred care.
This is a permanent opportunity, 24 hours a week, with a starting salary of £18,000 FTE.
In order to be successful, you will have previous care planning experience, ideally within a nursing home. You must have experience in administering medication and organising activities would be an advantage.
It would be advantageous if you hold an NVQ Level 3 in Health & Social Care, although this is not essential.
